Founded in 2025 as a small, independent practice in Northwest Indiana with a clear purpose: Finding the missing piece to close the gap between addiction and mental health care.

Built by a dual-certified nurse practitioner with more than 30 years in health care, the practice grew out of firsthand experiences with how trauma, addiction, and untreated mental illness can devastate families.

Personal Wellness Medical provides trauma-informed addiction treatment, psychiatric care, and crisis bridge services to adults seeking real, long-lasting recovery.
